Goal: The goal of Healthy Families Palm Beach is to prevent child abuse and neglect.

Impact: The Healthy Families program improves birth outcomes, nurtures child development, prevents child abuse and neglect, improves family functioning, and help parents develop more positive beliefs in their parental roles.

Goal: The mission of the Healthy Kids Express Asthma Program is to provide quality asthma care and education to children with asthma and their caretakers in a school environment to work towards goals set by Healthy People 2020 and the National Asthma Education and Prevention Program (NAEPP).

Impact: The Healthy Kids Express Asthma Program serves approximately 600 children a year, improving inhaler technique and asthma knowledge among participants. Children enrolled in the program for two consecutive years have lower school absentee rates and hospitalizations due to asthma.

Goal: Healthy Love seeks to provide a safe, culturally tailored intervention for heterosexual black women to reduce their disproportionately high risk of transmitting and contracting HIV and other STDs. Healthy Love aims to encourage sexual abstinence, HIV testing, and receipt of test results; increase women's condom usage during vaginal sex with male partners; and reduce the number of women's sex partners and unprotected anal and vaginal sex with male partners. Healthy Love also seeks to improve HIV/STD knowledge, self-efficacy for using condoms, intentions to use condoms, and attitudes towards condoms.

Impact: Healthy Love increased participants' likelihood of using condoms, being tested for HIV, and receiving their test results. The intervention also reduced participants' self-described actions with male partners that can increase black women's risks for HIV infection.

Goal: The goal of Healthy Relationships is to reduce the risk of HIV transmission by people who know they are HIV-positive by reducing the frequency with which they have unprotected sex.

Impact: Healthy Relationships seeks to develop decision making and problem making skills to reduce the risk and transmission of HIV through behavioral intervention. An intervention study resulted in significantly less unprotected intercourse and greater condom use with lower estimated HIV rates post-intervention.

Goal: The initiative's primary purpose was to reduce infant mortality by 50 percent and generally improve maternal and infant health in at-risk communities.

Impact: 20% of the Healthy Start program sites had significantly lower rates of low-birth-weight babies than their comparisons. 20% of the sites also had significantly lower rates of very-low-birth-weight babies than their comparisons. Four of the sites had significantly lower pre-term birth rates.

Goal: The Pennsylvania Department of Aging offers this fall risk screening and prevention program to adults 50 years of age and older. The program is designed to raise awareness of falls, introduce steps on how to reduce falls, improve overall health, and provide referrals and resources.

Goal: Improve early identification of concerns and initiate interventions to improve the health, development and emotional wellness of children, newborn to age three.

Impact: HSFYC parents were less likely to use severe discipline (OR: 0.68) and more likely to negotiate with their child (OR: 1.20). HSFYC parents had greater odds of reporting a clinical or borderline concern regarding their child's behavior (OR: 1.35).

Goal: The goal of HELPS Programs is to strengthen students’ reading fluency so they will be better able to focus on and improve other important reading skills, including comprehension.

Impact: HELPS is a supplemental curriculum that improves students reading fluency, a commonly neglected aspect of children's core reading curriculum, in order to help them become successful readers.

Goal: To educate the Chinese Canadian immigrant community on Hepatitis B knowledge and to promote Hepatitis B testing through ESL curriculum.

Impact: The Chinese immigrant population in North America has a rate of Hepatitis B infection that is 10 times higher than the general population's rate of approximately 0.5 percent. The Hepatitis B ESL curriculum is effective in increasing knowledge about Hepatitis B among Chinese immigrants in Canada.
