Goal: The goal of Saving Smiles is to improve the oral health and in turn overall health of children in Lafayette County, especially for the poor and underserved.

Impact: Most students from the 11 participating schools in Lafayette County received a screening, fluoride varnish, and oral health education, and most of the children referred for dental care were seen by a dentist.

Goal: The goal of this project is to educate citizens, health professionals, researchers, and policy makers about cancer in Missouri. Complete, timely, and high-quality data are essential for conducting research and responding to public concerns about cancer incidence in their communities.

Goal: Global Gardens aims to increase household access to affordable fresh vegetables among newly resettled refugee individuals living in the Historic Northeast of Kansas City, Missouri. Program participants are provided use of land, water, seeds, and plant starts at community gardening sites in the Lykins and Indian Mound neighborhoods, maintained by Global Gardens staff.

During the course of the program, growers receive ongoing education and support from staff and interpreters, reducing language and cultural barriers that have historically limited refugee access to community gardening engagement. The Global Gardens curriculum focuses on increasing refugee growers’ skills in community and household gardening, utilizing a garden-based learning theory of education, and implementing participatory, learner-centered assessment techniques. The curriculum empowers growers to take the lead in their learning experience and increase connection to and responsibility for their physical environment.

Additionally, Global Gardens aims to increase participant knowledge of how to access local community gardening resources in Kansas City. Each growing season, participants are connected to Kansas City Community Gardens (KCCG), a non-profit that seeks to assist low-income households to produce vegetables from garden plots in backyards and community sites. Global Gardens participants receive membership information and introduction to the seed and plant ordering process, and practice using this resource during the course of the program, building individual self-sufficiency in navigating the process, and increasing likelihood of utilizing KCCG in future.

Goal: To provide transportation to patients in South Central Missouri who otherwise have non-existent, limited, or expensive transportation options to and from healthcare appointments.

Impact: HealthTran has helped to improve healthcare access and long-term health outcomes, as well as reduced preventable hospitalizations and unnecessary emergency department visits.

Goal: The mission of the Healthy Kids Express Asthma Program is to provide quality asthma care and education to children with asthma and their caretakers in a school environment to work towards goals set by Healthy People 2020 and the National Asthma Education and Prevention Program (NAEPP).

Impact: The Healthy Kids Express Asthma Program serves approximately 600 children a year, improving inhaler technique and asthma knowledge among participants. Children enrolled in the program for two consecutive years have lower school absentee rates and hospitalizations due to asthma.

Goal: The goal of this program is to improve colorectal cancer screening rates among older adults.

Impact: Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

Goal: ERP’s main objective is to provide therapeutic services to youth in the juvenile justice system and their families while maintaining a commitment to public safety.

Goal: The goal of this program is to educate during every visit, to assist patients in developing a longitudinal personal record of medical history and care plans, and to provide a tangible way to engage patients in their own care. The overarching goal was better compliance, recognition of medication side effects, and improved adherence to specific and agreed upon lifestyle changes.

Impact: Sixty percent of patients participate in care plan tracking with a health notebook, and 80% percent of patients complete a prep form to help organize visits.

Goal: The mission of HKES&I is to increase access to needed healthcare for children. This is accomplished through the goal of identifying children who need further follow-up, particularly for hearing and vision problems, and to connect them to those services.

Impact: Healthy Kids Express Screening and Immunization program is successful in providing evidence based care to low-income populations and for connecting those identified with screening needs to needed follow-up care.
