The INC Well

A Good Idea

Description

"The INC Well" is an organic approach to addressing the chronic health disparities in the urban core community. The program utilized an existing resource - in this case a storage area - and repurposed it into a permanent, dedicated space. Residents are able to visit this space during business hours for blood pressure screening, health resource materials, and healthy snacks. Once a month, a certified health representative (i.e., a physician, nurse, nurse practitioner, or HEAL Advocate) is on site to offer assistance and to help support healthy lifestyle choices.

This outreach has been funded by the Black Community Fund and the American Heart Association.

Goal / Mission

The INC Well's goal is to improve upon the built and social environment to establish a health and wellness supportive presence for those who are taking a proactive role in their quality of life.

Impact

The INC Well created a dedicated space where people can utilize health resources and take their wellness into their own hands.

Results / Accomplishments

The INC Well created a database of those who are practicing self-management health practices, presented outreach opportunities for those who are uninsured and underinsured, and encouraged healthy eating and active living.
